This page centralizes the evaluation forms for all internships registered with AUP. Please follow the instructions below to submit the correct evaluation form. 
 

EVALUATION FORMS TO BE SUBMITTED BY THE FACULTY READER 

Professors should submit the relevant evaluation form to grade the student’s internship assignments.

• MA 4-CR Faculty Reader Evaluation for Internships 

EVALUATION FORMS TO BE SUBMITTED BY A STAFF MEMBER 

• Bachelor's 0 or 1-CR Internship Grade Change Form 
• Bachelor's 2-CR Internship Grade Change Form 

• Master's 0-CR Internship Grade Change Form 
• MA 6-CR Faculty Reader Evaluation Form 
• MAGC First and Second Reader Evaluation Form
